Change of Address Policy and Procedure

According to Section 27-7-17(B), Code of Alabama
1975: "Licensees shall inform the commissioner by any means acceptable
to the commissioner of a change in legal name or address within 30 days of the
change. Failure to timely inform the commissioner of a change in legal name or
address shall result in a penalty of fifty dollars ($50)."

| "Address Change Request (ACR) provides a
convenient method for producers to accurately communicate a vital piece of
information to regulators. It also, at no cost and in one place, allows
producers to view all their current address records for every state, making it
easier for them to monitor and maintain their records." |

There are currently 45 jurisdictions participating in ACR. The remaining six
jurisdictions will be added by the end of the second quarter of 2008. Producers
and authorized users can view the complete list of participating states and
submit individual producer address changes through the
NIPR web site.

| AS OF MARCH 1, 2008 THIS PROCESS WILL BE
MANDATORY FOR ALL PRODUCERS, SERVICE REPRESENTATIVES AND ADJUSTERS. However,
producers with Legal issues and Business Entities will have to continue to use
the Address Change Form. |
